Assisted Living services at The Heritage at Fountain Point are dedicated to providing assistance, as needed, with all activities of daily living (ADLs). Multiple Levels of Care supply an array of personalized services designed to meet your changing needs.
A points-based needs assessment by our health services team determines your individualized service plan of wellness and care. Multifunctional clinical/operations team reviews changes in your condition. Services include:
- Bathing/showering, grooming and dressing available as needed
- Dining, including food preparation and delivery of meals to an apartment
- Ordering, set up and administration of medications, inhalers, injections, eye, ear, nose drops, nasal sprays and oxygen
- Blood sugar checks and insulin injections
- Toileting and incontinence care
- Getting in and out of a chair or bed
- Mobility assistance
- Scheduled transportation service
- Whirlpool bath available or shower
- Support stockings, braces, splints and prosthetics
- Coordinate care and supplies with other medical providers

What Is the Difference Between Assisted Living and Skilled Nursing Care?
The main difference between Assisted Living and Skilled Nursing is the type and extent of care provided.
Assisted Living communities provide assistance, as needed, with activities of daily living.
Skilled Nursing communities (SNCs or SNFs) provide both short-term and long-term rehabilitation and medical care. Some stand-alone Skilled Nursing communities are affiliated with large hospital networks. (In the past, people often referred to SNCs as “nursing homes,” but that term is largely obsolete.)
For the most part, care in Assisted Living communities is provided by certified nursing assistants (CNAs) and medication management technicians (med techs). While most Assisted Living communities will have a supervising registered nurse (RN) or licensed practical nurse (LPN) as part of the care team, they generally provide supervision and oversight, not daily practical nursing care.
The majority of care provided in a Skilled Nursing community is nursing care delivered by LPNs or RNs. Care is often required 24/7 and is considered high-level nursing care.
