CANTAB Brain Health Assessment

CANTAB is a clinically validated, computerized brain health assessment — backed by research and used in clinical settings worldwide. In just 15–20 minutes, it evaluates memory, attention, and other key cognitive functions, giving your care team an early, accurate picture of your brain health. Covered by most insurance, the assessment includes a personalized review and recommendations to help you stay sharp, independent, and confident. An initial assessment is offered within 30 days of move-in, with annual reassessments as needed.

Palliative Care

Living with a serious or chronic illness is hard. Palliative care is about making it a little easier, providing specialized symptom management, emotional support, and care coordination that works alongside your existing providers, not instead of them. Our nurse practitioners collaborate with you, your family, and your healthcare team to focus on what matters most: your quality of life, your comfort, and your peace of mind. Palliative care can begin at any stage of an illness, and it’s covered by Medicare Part B and many insurance plans.

Advance Care Planning and Goals of Care

The best time to talk about your future healthcare wishes is now — before a moment of crisis requires someone else to make those decisions for you. Through the Five Wishes program and ongoing Goals of Care conversations, we walk alongside you in documenting your preferences, choosing a trusted decision-maker, and making sure your voice is part of every care decision. It’s one of the most thoughtful things you can do for your loved ones — and for yourself.

Hospice Care

When the focus shifts from curative treatment to comfort and quality of life, our hospice team is here for you, and for your family. Hospice care is about honoring who you are and how you want to live. Our team of nurses, social workers, spiritual counselors, and volunteers provides pain and symptom management, emotional support, and practical assistance, wherever you call home. Care is covered by Medicare, Medicaid, and most private insurance plans.
